SB 5201 Langerhans cell histiocytosis
Diagnostic Talley: Histiocytosis—2, granulomatous disease—25, Crohn’s—1.
IPOX: CD68(+), S100(+), CD1a(+).
SB 5202 Histoplasmosis + Candidiasis
Diagnostic Talley: Histoplasmosis + Candida infection—unanimous.
Cultures positive for above organisms.
SB 5203 Intravascular ALK-1 (+) anaplastic large cell lymphoma
Diagnostic Talley: Rosai-Dorfman disease—5, inflammatory lesion—2, intravascular lymphoma—15.
IPOX: CD30 (+), ALK-1(+), CD2(-), CD56(-), CD79a(-), EBV(-).
SB 5204 Acinar cell carcinoma
Diagnostic Talley: Large cell NE carcinoma—10, acinar cell carcinoma—10, mixed acinar+endocrine carcinoma—3.
IPOX: A1ACT (focally +), all neuroendocrine and peptide hormone markers negative.
SB 5205 Neuroendocrine cells within adipose tissue, favor heterotopic pancreas but can not rule out carcinoid (primary vs metastatic).
IPOX: Chromogranin (+), synaptophysin (+), pankeratin (+).
Diagnostic Talley: Vascular malformation—3, NE tumorlets—5.
SB 5206 Extramedullary myeloid cell tumor, consistent with acute myeloid leukemia with abnormal eosinophils.
Diagnostic Tally: Granulocytic sarcoma—20, lymphoma—2.
IPOX: MPO(+), CD34 (subset of blasts +), CD19 (-), CD56 (-),
SB 5207 Metastatic melanoma, primary site unknown
Diagnostic tally: Melanoma—unanimous.
SB 5208 Prostatic stromal proliferation consistent with low grade spindle cell neoplasm.
Diagnostic Tally: Solitary fibrous tumor—5, stromal hyperplasia secondary to Finasteride therapy—20.
IPOX: CD34, keratin cocktail, SMA, S100, Ki-67 (all negative).
Consultants: Stanford: Low grade spindle cell neoplasm. AFIP: Stromal hyperplasia.

Diagnostic Talley: Parathyroid adenoma, water clear cell variant—unanimous.
SB 5210 Glomangiopericytoma
Diagnostic Talley: Hemangioma—2, spindle cell myoepithelioma—1, monophasic synovial sarcoma—2, glomangiopericytoma—25.
