SB 5061 Cutaneous Acanthamebiasis, Skin.

Diagnostic Talley: None (diagnosis inadvertently revealed)

follow-up: The patient is doing well following treatment.

SB 5062 Phaeomycotic Cyst, Soft Tissue of Right Foot.

Diagnostic Talley: Mycetoma-2, pigmented fungus/chromomycosis-44.

Follow-up: The patient is lost to follow-up.

SB 5063 Dendriform Ossification, Right Lung. Diagnostic Talley: Osseous metaplasia-28, alveolar ossification-l,

Dendriform ossification-10.

SB 5064 Poorly Differentiated Squamous Cell Carcinoma, Hard Palate.

Diagnostic Talley: Adenocarcinoma ? Acinic cell carcinoma/clear cell carcinoma-6,

lichen planus-2, inflammatory, ? Sjogren's-4, carcinoma NOS-4.

Follow-up: The patient is well after resection.

Note. The tumor cells are positive for keratin, HMW keratin and p63. The subsequent

excision showed typical keratinizing squamous cell carcinoma.

SB 5065 Small Lymphocytic Lymphoma/Chronic Lymphocytic with Large Cell

Follicular Lymphoma, Skin.

Diagnostic Talley: Lymphoma-unanimous.

Follow-up: The patient is well at one year following chemotherapy although there is

residual thrombocytopenia and leukocytopenia.

Note. This patient's bone marrow and left axillary lymph node both demonstrate classic

findings for small lymphocytic lymphoma/CLL.

SB 5066 Intracranial Enterogenous Cyst, Fourth Ventricle, Cerebellum, Brainstem and

the Superior Aspect of the Spinal Cord.

Diagnostic Talley: Enterogenous cyst-unanimous.

Follow-up: The patient underwent partial resection and still has cervical spine symptoms at

one year.

SB 5067 Metastatic Carcinoma with Estrogen Receptor Expression, Skin of Temple. Diagnostic Talley: metastatic carcinoma (breast, ovary, endometrium)-unanimous. Follow-up: The patient remains asymptomatic and is well at one year without additional treatment and with no additional lesions.

Note: Tumor cells Her2 neg and weakly PR positive. TT Fl and WT-1 are negative. All studies on this patient have been negative including physical examination, MRI and a biopsy of a slightly thickened area on mammogram.

SB 5068 Mixed Epithelial Carcinoma, Endometrioid and Transitional Cell Type, Grade

2, Right Ovary.

Diagnostic Talley: Sertoli-Leydig cell tumor-6, sex cord stromal tumor-4

endometrioid carcinoma-14, neuroendocrine carcinoma-3.

Follow-up: The patient is well at one year.

Note: Inhibin- negative, EMA -positive, Cam 5.2- strongly positive CDX -2 -negative

WT-1 shows strong nuclear staining in the nodular areas.

SB 5069 Sebaceous Carcinoma Arising in an Adnexal Neoplasm with Follicular and

Sebaceous Differentiation, Occipital Scalp.

Diagnostic Talley: Pilomatricomal carcinoma-26, sebaceous carcinoma-1,

pilomatricoma-2.

Follow-up: The patient had a re-excision and is well at six months.

SB 5070 Myxoid Fibrosarcoma, Prostate Gland.

Diagnostic Talley: Pseudosarcomatous myofibroblastic proliferation-1, neurofibroma-12, stromal tumor of uncertain malignant potential-4, myxoid stromal nodule-6.
